Many people are unaware of the link between hypertension and diabetes, leaving them unprepared to prevent, recognize, or manage the condition. This project set out to change that by educating patients and their families, providing physicians with the latest treatment information, and connecting users with local healthcare professionals for guidance and care.

The work began by mapping content to specific user segments, tailoring resources for both healthcare professionals and non-professional audiences. These insights guided the creation of sitemap, user flows, wireframes, component definitions, and high-fidelity prototypes, featuring smart search, geo-detection, and condition-to-provider linking. Iterative usability testing ensured the experience was clear, trustworthy, and efficient, while metrics tracked engagement, search effectiveness, appointment bookings, and provider interactions.

The result was a personalized, seamless platform that increased awareness of hypertension in the context of diabetes, empowered preventative and diagnostic action, strengthened connections between users and their support networks, and improved access to local healthcare guidance.
